The Holdfast Bay Community Centre is a hub of activity, hosting over 30 community based programs every week. The Centre delivers a vast range of community strengthening programs including health and fitness, hobby and youth-focused programming.
The Commonwealth Home Support Program (CHSP) is a government funded service that supports older people to remain living independently at home. The Holdfast Bay Community Centre offers various CHSP programs from Monday - Thursday that will help you improve your health and wellbeing to support you to remain active and living independently at home.

Who can access CHSP Programs?
Over 65 years of age - Please phone MyAgedCare on 1800 200 422 and ask for a referral to Holdfast Bay Community Centre. We can support you in this process if required. If receiving another service, please check your Regional Assessment Officer for eligibility.
Living with a disability - NDIS approved programs for people living with a disability. YMCA is a provider of NDIS programs & eligible for plan funding depending on your goals.
Program fees start from $10.00 and extra $5.00 for transport. Fee support is available for those experiencing financial hardship.
Transport can be arranged for those living at home in the Holdfast Bay area. The Holdfast Bay Community Centre provides a 10 seater bus with rear wheelchair access. Bus is subject to availability.

Age should be no barrier to regaining your strength and balance. The Strength for Life (S4L) program is designed to assist people over 50 to access services that provide strength training programs suited to your needs and goals. The aims of the program are to encourage older people to engage in strength training and experience the benefits this provides to their well being and general lifestyle.
Please note: ALL Strength for Life Participants must attend a one-off assessment that is conducted by YMCA qualified fitness instructor to develop a personalised exercise plan prior to starting the program. This is at a cost of $30

How to Strength for Life
All participants require S4L forms to be completed before they call the Centre to book an assessment. This will include a pre exercise questionnaire which will determine whether a referral from the doctor is required. We also require an enrolment form to be filled out prior to activity. These forms can be found here by clicking on the links above or on the COTA SA website. Alternatively participants can call our centre on 8298 7422 to arrange for the forms to be mailed out to them.
** It is recommended that all participants intending to participate in S4L sessions visit their Doctor beforehand, so that your ongoing health can be managed appropriately.**
Referring a Client
If you are a local GP or Physiotherapist looking to refer your client into the Centre's S4L Program to support their health goals; simply complete a GP Referral Form or Physio Referral Form. Once complete, simply post it back to us here at the centre, alternatively via email at firstname.lastname@example.org or fax it through on 8377 4921. All clients are monitored by a qualified YMCA fitness instructor with S4L accreditation. All client programs will be individualised and progressive.
